Reporting On Asthma
The Empire Plan Report includes the June 2002 Reporting On Asthma. This publication provides information about asthma’s causes, warning signs and treatment and answers commonly asked questions about the condition. Reporting On Asthma will also be available as a stand-alone publication.
Distribution of Reports
The Reports will mail to enrollee homes in June and July. We will send you a supply of the Reports equal to 3 percent of your enrollment as we mail each version. We will send you a supply of the stand-alone version of Reporting On Asthma equal to 3 percent of your enrollment.
Publications available on this site
Please check this Web site for copies of new publications. We generally post publications when we sign off for printing. That means you can see the publications before enrollees start receiving them.

You may order additional copies of the Reports by using the Publications Supply Order Forms that was enclosed with this memo. Or, instead of faxing the forms, you may order from this site.
