SUBJECT: Dental and Vision Benefits While on Leave Without Pay
DATE: August 17, 2001

The Employee Benefits Division now provides dental and vision enrollment data to GHI and Davis Vision through NYBEAS transactions. Therefore, GHI and Davis Vision will no longer update an employee's file from information provided on Forms PS-816 or PS-817. For this reason, all transactions affecting employee benefits must be completed on NYBEAS.

With the recent production of the NYBEAS enrollment files, the procedures for continuing dental and vision benefits while on Leave Without Pay have also changed. It is no longer necessary to complete the PS-809 Form and submit premiums in advance of the leave. However, dental and vision benefits will now be terminated when an employee goes on Leave Without Pay unless the employee completes the PS-404 for continuation of dental and/or vision benefits and this transaction is processed on NYBEAS. Once the enrollment transaction has been processed, the employee will be billed for their dental and/or vision coverage. HBAs should no longer send advance premium checks to the Department of Civil Service Dental/ Vision Pending Account.
