Empire Plan
A Notice of Amendment to the General Information Book and Empire Plan Certificate has been mailed to all enrollees in the Empire Plan. This notice describes the Empire Plan’s uses and disclosures of Protected Health Information. It applies to all groups in the Empire Plan and is effective April 14, 2003.

Look for the new HIPAA Privacy Information section on PA-MARKET at www.cs.state.ny.us/ebdonline. Enrollees may also find this notice online at www.cs.state.ny.us by selecting Employee Benefits and then the new HIPAA/Privacy Information button.

Please give one to every new Empire Plan enrollee hired since March 5, 2003, the date the labels for this mailing were drawn from NYBEAS.
