SUBJECT: Correction to the PA Core Version of The Empire Plan At A Glance for 2005

We regret to inform you that the January 1, 2005 Empire Plan At a Glance for Participating Agencies with Core benefits contains an error that has required us to print a corrected version. The correction, to clarify that Core enrollees have no hearing aid benefit, appears on page 7. The revised Empire Plan At a Glance will be mailed to enrollees on the NYBEAS file on May 19, 2005 during the second week of June. We apologize for this error and any inconvenience it may cause.
Distribution
We are sending you a supply of the corrected 2005 Empire Plan At a Glance for Participating Agencies with Core Benefits equal to 5 percent of your Empire Plan enrollment for new hires or for those changing their option to the Empire Plan. Please destroy your current copies of the January 1, 2005 Empire Plan At a Glance for Participating Agencies with Core Benefits [PA0128 SB-PA Core-1/05].
The Publications Supply Request Forms have been updated to allow you to order the corrected 2005 Empire Plan At a Glance for enrollees with Empire Plan Core benefits [PA0130 SB-PA Core-5/05 (COR)]. You will receive a copy of the new form with your supply of the corrected publication. Or, instead of faxing the forms, you may order on PA-MARKET.
