The Empire Plan Across the United States and The Empire Plan Worldwide are now available to download and will be available to order in the coming weeks.
These publications explain how to access Empire Plan benefits while traveling or residing anywhere across the globe. Both include updated instructions on finding network providers and submitting applications for reimbursement.
These publications replace On the Road With The Empire Plan. Please recycle any existing copies.
Distribution and Access
Publications will not be mailed to enrollee homes, but they may be accessed on our website at nyship.ny.gov.
In the coming weeks, you may order copies on HBA Online under “Publications & Forms” using the following publication codes:
- The Empire Plan Across the United States (AL2100)
- The Empire Plan Worldwide (AL2101)
