The Empire Plan is a unique health insurance plan designed especially for public employees in New York State. Empire Plan benefits include inpatient and outpatient hospital coverage, medical/surgical coverage, Centers of Excellence for transplants, infertility and cancer, home care services, equipment and supplies, mental health and substance abuse coverage and prescription drug coverage.

It has come to our attention that PA CORE agencies are receiving shipments of both Empire Plan Copay Cards A and Copay Cards B.
Only Copay Card A (the blue card) applies to enrollees of Participating Agencies.
Please recycle the Copay Cards B that you have mistakenly received. The Copay Cards A are for distribution to your new hires.
