It was discovered that the Summary of Work-Related Injuries and Illnesses Form (SH-900.1 - 2004 +) inadvertently reported incorrect values for two of the fields in the Number Of Days section (4). Specifically, the values for Away From Work (Column K) and Job Transfer or Restriction (Column L) were transposed.
A fix to the form has been made. SH-900.1 forms generated during the past several weeks, including those generated earlier on January 20, 2026, may contain incorrect values and should be re-run to ensure accuracy. All reports generated after 12:00 p.m. on January 20, 2026, will contain the corrected data.
