The Employee Benefits Division (EBD) has updated the NYSHIP publications order forms available on HBA Online. You may now order benefits publications through one form for each enrollee type/group. Quick order forms are no longer available.
To access an order form, visit the HBA Online homepage, click “Publications & Forms,” choose a type of enrollee and choose a group (if applicable).
Benefits publications are organized by category. Some categories are specific to The Empire Plan and are indicated as such. Otherwise, publications apply to a variety of benefit types, including health maintenance organizations (HMOs), NYS dental and vision, and more.
To order publications, enter your requested quantities in the boxes on the right-hand side of any form, then click “Continue” on the bottom right to proceed. Only mark quantities for items needed.
CATEGORIES
- New Hire/Eligibility and General Information (NY, PA, PE Only). These essential publications are for prospective enrollees or enrollees wanting general information about benefits and options. We recommend ordering a newly benefits-eligible individual a copy of each publication listed. This section serves the same purpose that quick order forms did and more.
- SEHP Publications List (SEHP Only). This is a list of publications for prospective or current enrollees of the Student Employee Health Plan (SEHP). We recommend ordering a newly SEHP benefits–eligible individual a copy of the SEHP General Information Book and, if also a SUNY employee, the SUNY SEHP Rates Letter.
- Medicare and Retirement Publications. These publications are for retirees, active employees considering retirement, and anyone covered by NYSHIP who is (or will soon be) eligible for Medicare.
- Flyers. These publications are for distribution at benefits-related events to share information about specific topics.
- Empire Plan Overview Publications. These publications provide in-depth overviews of the benefits available through The Empire Plan.
- Empire Plan Special Publications. These publications cover specific Empire Plan topics such as mental health, prescriptions, how benefits work during travel, and much more.
- Workers’ Compensation (NY Active, SUNY SEHP Only). The Accident Reporting System (ARS) resources on this list provide information about how to report a work-related injury or illness.
DELIVERY ESTIMATES
Orders should typically arrive within 7 to 10 business days. Delivery times listed are estimates and may vary depending on holidays, time of year, volume of orders, shipping vendor delays or other factors. Plan for business needs accordingly.
For quick access and distribution, publications and forms are available in PDF format through HBA Online and the NYSHIP website (nyship.ny.gov).
