The notification requirements for National Medical Support Orders include notice to each of the affected insurance carriers. A carrier contact sheet was included as an attachment to HBA memo (NY 2004-02, PE 2004-03, PA 2004-05) to more easily facilitate this carrier notification.
Since the prescription drug vendor for the Empire plan has changed, the carrier contact sheet has been updated with the appropriate address and is attached for your use. Empire Blue Cross Blue Shield is insuring and jointly administering the new prescription drug program with Caremark, its pharmacy benefit manager. Please use this new contact sheet and discard the previous version.
