We are pleased to announce the newly revised Reporting On Prescription Drugs (July 2009). It replaces the July 2006 version and includes valuable information about The Empire Plan Prescription Drug Program. Reporting On Prescription Drugs applies to Empire Plan enrollees with Empire Plan Prescription drug coverage.
Distribution
This publication, including a mail service order form and envelope, will be mailed standard class to all Empire Plan enrollees with Empire Plan drug coverage in mid to late August. We will send you a supply of the standalone version (AL0859; without the mail order form) equal to one percent of your Empire Plan enrollment for new hires and new retirees.
