We are happy to announce the new M/C Life Certificate 2010 is at the printer. This book (AL0974) replaces (NY0551). Please recycle any old books on your shelves when you receive your supply.
To be eligible to participate in the M/C Life program, an enrollee must be:
- A Management/Confidential employee of the State or a Participating Employer
- A Member or Annual employee of the State Legislature; or
- A Judge or Justice of the Unified Court System of the State or a non-judicial employee of the court system who has been designated Management/Confidential.
Distribution
The new M/C Life Certificate 2010 will not be mailed to the homes of enrollees. We will send two books to each agency with M/C enrollment. If you need additional copies, please order them at HBA Online at https://www.cs.state.ny.us. Select Publications and Forms, and select the M/C publications order form.
The M/C Life Certificate is also included in a weekly mailing to all newly eligible M/C enrollees.
The M/C Life package includes the certificate and enrollment forms.

- choose "Employees",
- select State Government employees, then
- select Life Insurance under "Health Insurance and other employee benefits."
