We will be mailing the revised version of Reporting On Diabetes to your agencies. The May 2001 Reporting On Diabetes updates the May 1996 version. Recently updated diabetes information and Empire Plan benefits are included in this two-page publication.
Distribution of Reporting On Diabetes
You will receive a supply equal to 5 percent of your Empire Plan enrollment for distribution to interested enrollees. The publication may also be used for health fairs. We are not mailing this publication directly to enrollees. You may order additional copies as you need them.
