We are pleased to announce that the NYS Dental Plan Certificate of Insurance and Attachment to the NYS Dental Plan Certificate of Insurance documents for enrolled PE employees and their covered dependents are now available on HBA Online and NYSHIP Online. The NYS Dental Certificate of Insurance is evidence of dental coverage under the group contract that the State of New York currently holds with EmblemHealth, the Plan’s administrator. The Attachment to the NYS Dental Plan Certificate of Insurance provides information about dental insurance benefits specific to PEs.
Distribution
EmblemHealth will begin mailing these publications to enrollee homes starting in late summer/early fall. These documents will also be mailed to new enrollees joining the NYS Dental Plan throughout the year.
