IMPORTANT INFORMATION FOR EMPIRE PLAN ENROLLEES

Certificate Page in Empire Plan Publications

Effective January 1, 2000, UnitedHealthcare Insurance Company of New York became the insurer for administering Empire Plan medical benefits. The Empire Plan certificate amendment transferring the obligation of medical insurer from Metropolitan to UnitedHealthcare is included in the April 2000 Amendments mailed to enrollees' homes. It is also in the July 1, 2000 Empire Plan Certificate for Participating Agencies with Core plus Medical and Psychiatric Enhancements.

Whenever you distribute a NYSHIP General Information Book and Empire Plan Certificate to an enrollee, please be sure to include the April 2000 Amendments (with the exception of Core plus Medical and Psychiatric Enhancements) plus all other amendments and Empire Plan Reports issued after the date on the Book/Certificate. The enrollee will then have a complete package of NYSHIP benefit information.
