This memo is to notify you that your PA Monthly Billing Statement, dated 3/03/2001 was generated with an incorrect due date. The due date that appears on the statement for the April 2001 coverage shows 4/25/01 under the Due Date column, the correct due date should be 3/25/01. Payment should be received by the Employee Benefits Division on or before 3/25/01.
