2002 Empire Plan Telephone Number Cards

We are happy to announce updated Empire Plan telephone number cards. The cards provide telephone numbers to answer questions about Empire Plan benefits and requirements. They replace the cards issued in February 2001.

Distribution

We will send a supply of the revised telephone number cards equal to 5 percent of Empire Plan enrollment to each agency for handout to interested enrollees. Enrollees will receive the telephone number cards as an insert in the spring 2002 Empire Plan Reports that will be mailed to enrollees’ homes.

Reordering

To order more cards, please use the Publications Supply Request Form attached to this memo. Or, instead of faxing the form, you may order on PA-MARKET. From the homepage, open Publications and Forms, then open NYSHIP Publication Order Forms. Choose the appropriate group, then "Order Online" or "Print or View Form" and follow instructions.
