Beginning January 1, 2003, Empire Plan CORE + Medical enhancements will no longer be available as an option. Your agency has selected Empire Plan CORE Plus (all enhancements) for your benefit package.
A packet of materials will be sent to the homes of all of the enrollees in your agency. We have attached a copy of the cover letter that will be enclosed in the package, to the printed version of this memo.
We will also send you a small supply for new hires. When you need additional materials you may order them online at PA-MARKET or by using the attached Publications Supply Request Form that was attached.
You may want to keep one set of Empire Plan CORE + Medical enhancement publications on file for reference. Enrollees have until the March 31, 2003 claims deadline to file claims for services provided under Core + Medical in 2002. Please recycle the rest of those materials so there is no confusion about which materials to give new hires.
