The revised New York State Dental Plan Certificate of Insurance for employees of PEs who are enrolled in the New York State Dental Plan and their covered dependents is now available on the NYSHIP website and HBA Online.
This Certificate, which is effective beginning October 1, 2024 and replaces the June 2023 New York State Dental Plan Certificate and accompanying attachment and rider, is evidence of dental coverage under the group contract between New York State and Anthem Blue Cross, the plan’s administrator.
Distribution
The New York State Dental Plan Certificate may be accessed on the NYSHIP website at nyship.ny.gov. It will not be mailed to enrollee homes.
