Incident Detail
If the injured employee or another employee who witnessed the incident is the one reporting the incident, enter their social security number in the Reported by EmplID field and press Tab on your keyboard.
Note that the Reported by EmplID employee's name appears to the right of the field.
Another selection on the People (page) under People Connected to this Incident will determine whether the person reporting is also the injured person
or a witness to the incident.
If the incident is being reported by anyone not an employee of the agency, a new non-employee ID record may need to be added to the system. This may be done by clicking To add a Non Employee, click here.
ARS systematically assigns alternate ID's to non-employee reporters. Note that all zeros will automatically populate in the Non-Employee ID field. The Non-Employee ID will not be issued until all information has been entered and saved. To enter the non-employee data, press Add.
To enter the non-employee's name, click Edit Name.
Indicate the person's Prefix, First Name, and Last Name. Then, click Refresh Name.
Note that the non-employee's Display Name, Formal Name, and Name displayed to the right. If the name is correct, click OK.
Note that the non-employee's name appears under Name. Click on the Address page.
Click on Edit Address.
Enter the zip code under Postal and press Tab on your keyboard.
Note that the County, City, and State automatically populate. Add the non-employee's address under Address 1 and Address 2, and click OK.
Note that the Address will automatically populate. Click on Phone to add the non-employee's phone number.
Select the Phone Type, enter the Telephone number, and click OK.
Note that non-employee ID number is populated under Reported by Non-EmplID and the persons name appears to the right. If the non-employee is also a witness, another selection will be made on the People (page) under People Connected to this Incident.
Once the Incident Detail page is complete, select the People page to the right of the Incident Detail page.
