Modified Incident Reports
The Modified Incident Report can be produced by personnel and health/safety administrators to monitor the work-related accident experience of an entire agency. The results may be used to identify hazardous working conditions for the purpose of proactively developing abatement programs.
The report can provide the following data fields for incidents that occurred within a specified period of time:
- name of the injured employee;
- medical treatment received (Yes or No);
- lost time from work (Yes or No);
- incident number;
- date of the incident;
- code and description of the body part(s) injured;
- nature of injury code and description;
- source of injury code and description;
- accident type code and description;
- location of the accident;
- title of the injured employee;
- negotiating unit of the employee; and
- number of days before the accident was reported.
The report may be ran with all codes (body part, nature of injury, source of injury, and accident type excluding descriptions), only body part codes and their descriptions, only nature of injury codes and their descriptions, only source of injury codes and their descriptions, only accident type codes and their descriptions, or number of days before the accident was reported. All reports include a field for name, incident number, medical, lost time, location, title and negotiating unit.
The report can be sorted by any field and provide subtotals for each grouping within that field. The report will also provide a grand total.
Follow the instructions below to create and print a Modified Incident Report.
Within the Incident Reports folder, click on Modified Incident Reports.
Enter the five digit department ID number (Dept ID) of your agency and click Search.
Use the magnifying glass icon next to From Location to input the first location of a range of locations for the Dept ID already entered.
If you would like to run the report for all locations associated with the Dept ID, click the first location at the top of the page.
Use the magnifying glass icon to select the last location of a range of locations for the Dept ID already entered.
If you would like to run the report for all locations associated with the Dept ID, click the last location at the bottom of the page.
**Note** If you would like to create a report for only one location, select the same location for both the From and Thru Location.
Enter the date in which the report will begin to show incidents from.
Enter the date in which the report will show incidents ending on.
Use the magnifying glass icon next to Report Type to select which values will be inputted within the report.
Select the appropriate type of report.
Use the magnifying glass icon next to Sort By to select how the report is to be sorted.
Select the appropriate value in which your report will be sorted.
Once the report's parameters have been entered, it can be processed. Click the Run button to initiate the creation of the report.
Click OK.
Click on Process Monitor.
For this report, PSNT or PSNTE must be selected for Server and Crystal must be selected for Type.
The most recent report created will be displayed at the top of all requests under the Process List. Under Run Status, you will see Queued, Initiated, Processing, then Success. (You may need to click the Refresh button until Success is shown.)
Once the Run Status displays Success and the Distribution Status displays Posted, click on Details.
On the Process Detail page, click on View Log/Trace.
Click on the .PDF report under File List.
Your report will open in Acrobat/Adobe Reader for viewing. Click on the Printer icon at the top of the page to print the Modified Incident Report.
