On the Lost Time page, begin by clicking Yes or No to answer the question "Did the employee stop work because of his/her injury?"
If Yes is selected, enter the employee's Last Day Worked by using its calendar icon. Enter the employee's 1st Full Shift/Day Missed from work by using its calendar icon. Also, indicate if the employee lost more than or is anticipated to lose more than one week from work.
Note: If; Notice of Injury Date on the C-2 Detail1 (page) and 1st Full Shift/Day Missed are entered, then Date Employer Had Knowledge of Date of Disability is required.
Next, click Yes or No to answer the question "Has the employee returned to work?". If Yes is selected, enter the employee's first date they returned to work using the calendar icon that appears. Also, indicate whether they returned to work at Regular Duty or Limited Duty.
The employee's Date of Hire will automatically populate based on what is present within their personnel record.
If applicable, provide the Date Employer Removed Employee From Payroll using the calendar icon.
The Claim Number, Form Name, and Form Rcd Date will automatically populate within the SIF Claims box when a form is received from the New York State Insurance Fund. For instructions on how to review the SIF Determination forms, go to the Claims Management section of this manual.
If the employee returns to work at Limited Duty, a description of Work Restrictions may be added to Injury/Illness if it is applicable. In order to add the Work Restrictions, click on the magnifying glass within the Work Restrictions.
Under Search Results, select the appropriate Work Restriction Code or Description.
Next, enter the dates that the Work Restrictions will take place by clicking on the respective calendar icons.
Additional Work Restrictions can be added by clicking the plus sign.
A description of Modified Duties may also be added to Injury/Illness if it is applicable. In order to add the Modified Duties, click on the magnifying glass within the Modified Duties box.
Under Search Results, select the appropriate Modified Work Code or Description.
Next, enter the dates that the Modified Duties will take place by clicking on the respective calendar icons.
Additional Modified Duties can be added by clicking the plus sign.
**Note** Work Restrictions and Modified Duties are not required to be entered in order to verify or send the FROI (C-2).
Once the Lost Time page is completed, select the Personnel page to the right of Lost Time.
