PESH Reports
The NYS Department of Labor's Public Employee Safety and Health (PESH) Bureau within the Division of Safety and Health (DOSH) oversees workplace protection of public employees at the state and local level. PESH enforces safety and health standards, including the requirements set forth in the Commissioner of Labor's Rules and Regulations Part 801 (12 NYCRR Part 801).
According to the Rules and Regulations, Part 801 implements Labor Law, "which provides for recordkeeping and reporting by public employers" for the purpose of "developing information regarding the causes and prevention of occupational injuries and illnesses, and for making public periodic reports of work-related deaths, injuries, and illnesses."
One of ARS' many functions is to assist users create and print three of the reports required by the Labor Law cited above. Click on each link below (or in the side navigation menu) to find instructions on how to create and print the chosen PESH report:
- SH900.2 - Injury and Illness Incident Report (This link will send you to the FROI (C-2) printing instructions. The First Report of Injury (FROI, formerly the C-2 Employer's Report of Work-Related Injury/Illness, is an acceptable substitute for Form SH900.2.)

Click on Part 801 under the subheading Employee Injury and Illness Recordkeeping Information and Form. The SH901 Instructions for Recording and Reporting Public Employees' Occupational Injuries and Illnesses are also listed under the same subheading.
