Location
Start the Location page by determining if the incident occurred on the employer's premises or not. If the incident occurred on the employer's premises, click the checkbox next to Occurred on Employer Premises.
If the incident occurred on the employer premises, then click the magnifying glass icon to the right of the Location Code field.
Under the Lookup Location Code screen, select the correct location where the incident took place.
If the correct location code is not shown, enter the the corresponding department abbreviation in the Description field and press Lookup. (Example: Department of Civil Service = CS) Then, select the correct agency location where the incident occurred.
Please refer to the Smart Codes section of the manual for a listing of Agency Smart Codes.
Once the agency location has been selected, the agency name and address will automatically populate and become grayed out.
Enter a brief description of the exact location where the incident occurred in the Exact Location field.
**Note** In the case that the incident did not occur on the employer premises, all address values for where the employee was injured will need to be entered manually as accurately as possible. (Incident Address may not exceed 30 characters.)
Once the Location page is complete, select the Injury Event page to the right of Location.
