Pending Termination Letter
A Pending Termination Letter is produced to provide an employee with at least 30 days' notice of the effective date of their proposed termination. This notice should not be served more than 60 days before the proposed termination date.
Follow the instructions below to view and print the Pending Termination Letter:
Begin by clicking on Workforce Monitoring from the side navigation Menu.
Within the Workforce Monitoring main menu, click on the folder labeled ARS Reports.
Next, click on the folder labeled ARS Letters.
Click Rule 5.9 Pending Termination.
Enter the Incident Number OR EmplID and click the Search button.
Enter the date the employee is scheduled to be terminated within the Proposed Employment Termination Date field.
Click the magnifying glass to view the selections for the Termination Reason.
Choose Completion of One Cumulative Year or Finding of Permanent Disability.
Enter the phone number the employee should call for questions they may have involving their workers' compensation benefit. Then, click Run.
Click OK.
Click on Process Monitor.
The most recent report created will be displayed at the top of all requests under the Process List. Under Run Status, you will see Queued, Initiated, Processing, then Success. (You may need to click the Refresh button until Success is shown.)
Once the Run Status displays Success and the Distribution Status displays Posted, click on Details.
On the Process Detail page, click on View Log/Trace.
Click on the .PDF file under File List.
The Pending Termination Letter will open in Acrobat/Adobe Reader for viewing. Click on the Printer icon at the top of the page to the print the letter.
